2018-2019 Back to School Student Information Update Process

Instructions
  1. Click on the link to the Home Access Center (HAC). Use the login and password information located within the inside cover sheet of the summer mailer. Use the tutorial on the right to learn more about making updates in the HAC.

    • Update the Health Information

    • Update the Emergency Contact information

  2. Complete the following

    • Authorization for Emergency Medical Treatment

    • Voluntary Participation Student Accident/Health Insurance

    • 2018-2019 Parent/Guardian Acknowledgement

    • Health History

  3. Turn in the above four forms (included in the Back to School Mailer) to the AHS main office during the following times:

  • Monday, July 23 - Friday, July 27, 2018: 9am - 2pm (HAC support available)
  • Monday, July 30 - Friday, August 3, 2018: 9am - 2pm (HAC support available)
  • Monday, August 6 - Friday, August 10, 2018: 9am - 2pm (HAC support available)
