    Booking Request Form For AHS Staff Only

  • The purpose of this Booking Request Form is 1) reserve time for an event in the AHS PAC, and 2) to create an understanding of event needs between the PAC Theater Manager and the person requesting use of the PAC.

    In order to ensure that events are successful and that the PAC is kept in good working order, it's important that all parties using the facility have good working communication and that all parties know what is expected of them.

    The following pages will take you through the details of your event.

    You must submit this form a minimum of ten days prior to the proposed start date of your event. 

    Submittal is NOT a guarantee that you have the space reserved! The Theater Manager will contact you to confirm your reservation.
