Computer Use Contract

I ____________________________, understand that it is a privilege to use the computer equipment in the Aptos High School Applied Arts Department. Therefore, I agree to treat the equipment with respect. I understand that it is against this Computer Use Contract to make ANY modifications to the equipment including, but not limited to:

(keyboards, mice, etc.)

passwords, etc.

As a department, we encourage current and former Applied Arts students to use the computer lab to enhance their work for other classes. However, avoid using the equipment for personal use such as checking e-mail or playing games on the Internet.

The consequence for breaking any of the rules above is grounds for a minimum of two lunch periods of detention. Continuing to break the rules will result in loss of computer privileges.

I have read and understand each of the rules for using the Applied Arts Department computers. By signing this form, I am indicating that I understand the consequences for breaking any of the rules within this contract. STUDENTS WILL NOT BE ISSUED A PASSWORD UNTIL THIS FORM IS BROUGHT BACK WITH A PARENT/GUARDIAN SIGNATURE.
